Would the Moome card accept arbitrary resolutions & refresh rates (below the 250Mhz limit), or only specifically the ones listed in the HDMI support mode table?

My understanding is that any custom resolution/refresh rate can be done (up to the limits of the device). You will however have to pick/choose custom resolutions with special porches with an HTPC (using something like powerstrip) or with an advanced video processor like the Radiance - it can be tricky to get right as not every display likes what it is sent. If you search through this Moome sub-forum you'll find people discussing this.

With the Sony Switcher PC-1270 or PC1271 the card can be used to connect to any RGBHV device.
Or with the HTU-Board, five BNC-plugs, an external 12V power supply, and some soldering activity, it can be connected to any RGBHV device.
