Looks like something off a digital projector, definitely not a CRT. From the writing, it looks to be an old projector, probably 15-20 years old, but finding out a model number? Good luck, I don't think you'll have any success with that, as there were so many.

That's looks like a single element camera lens or filter. that's most likely from an older model commercial camera. Projector lens use multiple elements and are longer throw

I thought it looked like a camera lens too. Most camera lenses are multi-element, but maybe an older one...?
