I've reflowed soldering to the opamp (U2) and things seem to be OK right now. I'll get back to this later and will then run things and see happens during a time testing.

Thanks MP! That's positive. Good idea to run a little time testing - sometimes it would work at initial power up, but then not after leaving it overnight. Appreciate the help!
